Shuffle Error: MAX_FAILED_UNIQUE_FETCHES; bailing-out

First, long time no use MapReduce! Today I wasted some time figuring this out; "Shuffle Error: MAX_FAILED_UNIQUE_FETCHES; bailing-out".
